you need to manage input control by drag and drop according to use cases. just manage the all necessary fields in your website contact form.

for web to lead just follow the basic steps

• Form Name: name of the form (example : website contact form )
• Lead source : (example : website Visitor)
• Lead Status : (Example : hot)
• Message : message shown after form fill up.

after saving the form you will get the HTML code, you just need to paste that code into your website.
